Anxiety Shared Pathway Toolkit Now Available
Anxiety is one of the leading causes of hospital readmissions and crosses many diagnoses. A new toolkit developed on behalf of the Lake Superior Quality Improvement Network (QIN) will help transitional care units and home care agencies prepare staff to mitigate anxiety symptoms and offer integrated therapy programs to alleviate anxiety and improve outcomes.
The Anxiety Shared Pathway Toolkit focuses on using integrative health care to help treat anxiety. Integrative health care emphasizes a holistic, patient-focused approach to health care and wellness – often including mental, emotional, functional, spiritual, social, and community aspects – and treating the whole person rather than one physical symptom. It aims for well-coordinated care between different providers and settings of care.
A workgroup consisting of a variety of participants throughout the care continuum, from hospitals and transitional care settings to home care and hospice, Stratis Health and others, developed the toolkit on behalf of the Lake Superior Quality Improvement Network (QIN) Coordination of Care Initiative – west metro community.
