BI and CADI Waiver Providers: August 16 Deadline for Customized Living Survey
On July 8, 2024, DHS will send a letter and survey to MN–ITS mailboxes of Customized Living (CL) providers that serve people on the Brain Injury (BI) and Community Access for Disability Inclusion (CADI) waivers.
By August 16, 2024, providers should complete this survey to:
- Verify that their setting information is correct.
- Submit corrections.
DHS plans to use this information to publish a list of active and enrolled CL settings in the Community-Based Services Manual (CBSM). The list will help lead agencies and people who receive services:
- Determine what BI and CADI CL setting-specific limitations apply to a location.
- Ensure people receive correct information about CL settings.
- Determine if a setting meets the requirements to provide CL services to the person.
CL Settings Limitations
For more information about limitations, refer to CBSM – CL (including 24-hour CL).
