The Minnesota Department of Health (MDH) is conducting focused infection control surveys in Housing with Services (HWS)/Comprehensive Home Care communities.  

Surveyors are using the COVID-19 Action Plan for Congregate Settings that MDH posted in early April. 

We encourage you to review this document and confirm that you have actions in place in each of these areas: administrative, surveillance, education, hand hygiene, transmission-based precautions, PPE, environmental cleaning, cleaning equipment, inter-facility communication and transfers, and COVID-19 educational resources for staff and residents.
