The Affordable Care Act mandated that the Centers for Medicare and Medicaid Services (CMS) establish risk categories for Medicare enrollment to determine what level of scrutiny to give provider enrollment applications, which includes initial enrollment, change of ownership (CHOW) applications, and revalidations.
