The Deadline to Convert to Assisted Living Licensure is June 1

Time is almost up for current assisted living service providers to submit a conversion application to assisted living licensure. After June 1, a provider needs to apply for a new or “provisional” license with additional standards applied and no assurance that the license will be approved. Unlicensed settings are prohibited from providing assisted living services after Aug. 1.

MDH will work with applicants if they have made a good effort to complete the applications and have a missing item or error, but the deadline of June 1 for applying is firm.

Notices must be provided to residents and others in several circumstances. Any comprehensive home care provider not planning to provide home care services after Aug. 1 must give written notice by May 31 to every resident who receives services. Likewise, a housing with services provider which does not intend to continue to offer housing after Aug. 1 must notify tenants in writing by May 31.

Notice requirements related to the Comprehensive Home Care and the Housing with Services registration can be combined into one letter. The notice must include:

For home and community-based waiver services recipients, providers must also notify the resident's case manager while providing notice to the resident.
