Connected Communities

Grant funding that enables providers to generate innovative solutions that aim to place seniors and their families at the heart of a community conversation, ensuring their needs and preferences guide a collaborative care network.

Pharmacogenomics Pilot Program

Pharmacogenomics, the study of how genetic variations influence an individual’s response to medications, presents a promising avenue for improving medication management. This funding is available for nursing homes and assisted living organizations interested in implementing pharmacogenetic testing in their facilities.

Grant Grid: Funding Opportunities for Aging Services Providers

A comprehensive grid of over 30 grants, scholarships, loans, workforce development programs, and more funding opportunities for Minnesota senior care providers to consider.

New Americans in Long-Term Care

We received a $1 million grant to help to bridge pathways between New Americans and senior care employers.
