Connected Communities Toolkit

The Connected Communities Toolkit offers a practical roadmap for bringing healthcare providers, nonprofits, businesses, local government and residents together to better support older adults. LeadingAge Minnesota members can use its tools and lessons to identify community needs, build stronger partnerships and create locally driven solutions that help people age well.

Organized around three stages—forming, transforming and sustaining—the toolkit includes guidance for assessing readiness, mapping community assets, engaging partners, measuring progress and planning for long-term sustainability.
