PURPOSE

Skilled Nursing operators and providers have been preparing for anticipated changes of claim based and coding-based measure updates. Now that changes are final, it is time to evaluate how the new 5 Star Quality Rating System and Quality Reporting Program (QRP) measures will impact your outcomes. The presentation will include value-based care discussions with suggestions for collaborations across settings and technology innovation that will focus on resident Health vs. Healthcare.


LEARNING OBJECTIVES

•Summarize the measure updates and understand the SNF QRP Discharge Self-Care, Discharge Mobility, and Discharge Function measures..

•Understand the SNF QRP increased data thresholds for MDS submissions and the SNF Value Based Purchasing adjustments.

•Identify steps for updating current data collection and outcome tracking with focus on patient engagement and behavior changes.

Nursing Home Administrators, Directors of Nursing, Business Office Staff, MDS Professionals ,and roles responsible for MDS assessments such as Rehabilitation Therapists, Social Servicesand Activity/Therapeutic Recreation.
