Posted on October 2, 2018 by Jeff Bostic

The Minnesota Board on Aging and Minnesota Department of Health will host a free webinar on Oct. 18 on how section Q of the MDS is used to target state resources towards assisting those nursing home residents who wish to return to a community setting.

The presenters will cover the following questions:

  • How do I complete MDS Section Q when the person has dementia?
  • What happens when I indicate I need to make a referral to the Local Contact Agency?
  • Who is the Local Contact Agency and does a referral automatically get made to them?
  • How is Senior LinkAge Line® involved?
  • What if the county is already working with the resident to go home?
  • How do I explain to the resident why I’m asking these questions?
  • What should the resident expect when I make a referral?

Pre-registration is not required. Members should use the link and information below to participate in the webinar. The webinar will be recorded for those who cannot participate in the live event.
