Health Dimensions Group® Celebrates 25 Years of Dedication to Seniors

Health Dimensions Group® (HDG® ), a leading senior living, post-acute, and long-term care management and consulting firm based in Minneapolis, MN, is proud to celebrate its 25th anniversary in 2025, a milestone marking a quarter-century of empowering seniors across the nation to live their best lives.

Quality Care, Service, and Consulting

Founded in 2000, HDG’s mission is to make lives better by providing quality care, service, and consulting. HDG manages 52 senior living and care communities in nine states, caring for more than 3,000 residents. Serving a broad range of clients in the aging services sector across all 50 states, HDG’s consulting division provides customized solutions tailored to each client’s unique needs from strategy development, operational performance improvement, and interim leadership to turnaround support.

“The senior living industry is constantly evolving, and we must evolve along with it in order to best serve our residents, their loved ones, clients, and partners,” Erin Shvetzoff Hennessey, Chief Executive Officer and Principal of Health Dimensions Group said. “Our dedication to serving seniors has been the foundation of HDG’s growth over the years. HDG is proudly a privately held, women-owned organization led by Amber Rogotzke and Erin Shvetzoff Hennessey. Under their leadership, HDG’s management and consulting practice has grown steadily, garnering acclaim as a premier aging services organization. A Growing Portfolio

Over the past two and a half decades, HDG has consistently grown its portfolio of managed communities. The Enrichment Beyond Expectations™ (EBE) program is at the heart of HDG’s management proven process, known as Caring Above and Beyond® (CAAB), serving as a transformative journey designed to enhance every facet of their residents’ lives. HDG is also expanding its Dimensions Living® brand of senior living communities, quickly becoming a mainstay in the industry.

The organization has also steadily grown its consulting practice, with particularly large advances in the strategic planning, operational and clinical consulting, and interim leadership and executive search service lines – and now, the organization is poised to take some of its biggest steps forward. As interest in the Program of All-Inclusive Care for the Elderly (PACE) continues to rise, HDG is expanding its PACE team and service offerings to meet the evolving needs of our clients.
