Mark Your Calendar: May 4 Coaching Room for Assisted Living Licensure Applications

On May 4 from 3-4 p.m., LeadingAge Minnesota will focus the regularly occurring Coaching Room on applications for assisted living licensure and the Uniform Checklist Disclosure of Services, a component necessary for application.

During the hour-long webinar, we will review the assisted living licensure application, which is expected to be posted by May 1. Additionally, we will discuss the Uniform Checklist Disclosure of Services required in the application process. The Uniform Checklist Disclosure of Services, in essence, replaces your current Statement of Home Care Services and the Uniform Consumer Information Guide and must be given to prospective residents. The Uniform Checklist has been developed by the Minnesota Department of Health and is also expected to be available on May 1.

We will briefly walk through need-to-know information about the applications and leave the remainder of the time for your questions.
