Perham Health’s “Elevate” Wins Rural Health Award

Congratulations to Perham Health/Elevate for winning the 2025 Minnesota Rural Health Team Award. This award, presented by the Minnesota Department of Health, recognizes dedicated health care organizations that have made significant contributions to the health and wellbeing of their rural communities. It's presented just in time for honoring Rural Health Day, which is today, Thursday, November 20. 

Elevate by Perham Health is one of the original sites that received funding from the LeadingAge Minnesota Foundation's Connected Communities initiative. Perham Health's Elevate program is being honored for its work to redefine what it means to age well in rural Minnesota. Elevate brings together health care and community resources to help older adults live with purpose, health and joy.
