Assisted Living Report Card Updates

On Feb. 12, the Department of Human Services (DHS) published new ratings and corrected previously published ratings for its Assisted Living Report Card.
New ratings
DHS published:
- New resident quality of life and family satisfaction survey ratings from surveys conducted by Vital Research between April 1 and Nov. 30, 2025.
- New health, safety, and staffing ratings from surveys conducted by the Minnesota Department of Health (MDH) between June 1 and Aug. 31, 2025.
- Links for consumers that will take them directly to State Licensing & Investigation Results from the past 12 months. This is found on each AL’s individual report card page, regardless of whether the facility was found responsible.
Corrected ratings
DHS corrected the resident health, safety, and staffing ratings for 268 assisted living settings after identifying errors as part of ongoing quality assurance efforts. DHS notified the affected assisted living providers by email on Feb. 12. A link to a list of providers with corrected data can be found here.
MDH collected the data, and the University of Minnesota (UMN) used the data to calculate ratings for the Assisted Living Report Card. The corrections are due to errors in previously published ratings and do not reflect provider performance.
MDH and UMN found several causes for the errors, including incorrect data entries and calculation mistakes across different datasets and survey years. They partnered with DHS to publish corrected ratings and resolve the source of the errors.
