Care Centers: Improve Your 5-Star Rating and Sustain Your Outcomes

As part of our ongoing commitment to help you improve performance and measure and report outcomes in your organization, LeadingAge Minnesota is proud to provide you with access to a new tool that will help translate your current MDS 3.0 data into actionable information. 

Quality Apex was designed to help care centers more accurately report their clinical data to CMS, analyze your performance against your organization’s goals and identify QAPI projects. Through the data analytics provided by Quality Apex, you will have the information you need to sustain or improve your 5-Star rating. 

We first shared news of this tool a few months ago. When several of our care center members previewed Quality Apex, they found the tool to be easy-to-use and timely. As it uses your most current MDS data upload, you have real-time access to key data to report outcomes, identify improvements and help ensure everyone on your team is on the same page when it comes to improving quality and sustaining the outcomes you achieve.
