DHS Updates MnCHOICES Fact Sheet

The Minnesota Department of Human Services has updated the MnCHOICES Fact Sheet. 

MnCHOICES uses a person-centered planning approach to help people make decisions about long-term services and supports. It replaces the Long-Term Care Consultation Tool, as well as the assessment tools previously used for Developmental Disabilities Screening and Personal Care Assistance Assessment, for purposes of determining the Case Mix Classification for Elderly Waiver funding availability. 

The single, comprehensive assessment and support planning web-based application is used by professionals who the state has certified as assessors and support planners for long-term services and supports. It incorporates the same ADL and other dependency indicators as the legacy Long-Term Care Consultation.

Currently all Minnesota counties and two tribes are using MnCHOICES, and efforts are being made for managed care organizations to use MnCHOICES. For more information, MnCHOICES Fact Sheet.
