Check Your Email: MDH Now Sending Home Care Survey Results by Email

Effective July 1, the Minnesota Department of Health (MDH) Home Care and Assisted Living Program discontinued mailing paper copies of license survey results to providers and began emailing the results to the agent on record with MDH. 

Minnesota statute 144A.472, subdivision 6 requires that providers update MDH within 10 days of any changes to the license, including changes to the agent or the agent’s email address. Failure to update MDH may interfere with the provider receiving important licensing and survey documentation and result in expired licenses and/or fines.
