Update: MDH Sending Survey Results Via Email to State Licensed Only Home Care Providers

Effective July 1, 2018, the Minnesota Department of Health (MDH) Home Care and Assisted Living Program discontinued mailing paper copies of survey results to state licensed only home care providers and began emailing the results to the agent on record. This change does not apply to Medicare certified home health agencies.
Providers must update MDH within 10 days of any changes to the license, including changes to the agent or the agent’s email address. Failure to update MDH may interfere with the provider receiving important licensing and survey documentation and result in expired licenses and/or fines.
